Appointments
It is our inte​ntion to provide your children with the best care possible at all times and to accommodate as many requests as is realistic and feasible. It is within this context that we ask you to take a few moments to review policies that affect the way services are provided.

  • ​​Patients are seen by appointment.
  • ​Appointments for additional children should be made by phone prior to coming to the office.
  • ​Arrive early for administrative requirements to be processed.
  • Please remember that all insurance must be    updated and verified prior to the visit.
  • Bring your insurance or Medicaid card with you to every visit.
  • New patients are required to complete registration forms.
  • Print the Patient Registration Form on this website, complete and bring to visit.
  • Established patients are required to complete an updated registration form annually.
  • Co-payments must be paid prior to visit.
  • Check your insurance plan for deductible amounts, preventive and immunization limits.
  • Call ahead if you are late or unable to make your appointment time.
  • Turn off cell phones in the office and examination   rooms.​
When to Schedule an Appointment

  • ADD / ADHD medications
  • Animal bites - unless no breakage of skin or swelling​
  • Antibiotics
  • Burns
  • Coughs
  • Eyes/Pink eye
  • Ears
  • Fever​​
      - Under 3 months of age – any fever
      - Infant ---------------------over 100.5
      - Children------------------not acting normal
  • Head trauma
  • Insect bites - if infected
  • Pain or burning on urination
  • Possible fracture
  • Rashes
  • Seizure
  • Sore throat
  • Wheezing/Difficulty breathing
  • EPSDT or HealthCheck exam
  • Physicals – including Sports Physicals
  • Immunizations
